The school may also select a student for verification.

Submission of Requested Documentation
All requested information must be provided to The Sandusky Career Center Financial Aid office within two weeks of the notification date. If the requested documentation is unavailable at the time of the request, an extension can be granted provided the Financial Aid office receives a written request explaining the reason the documentation is not provided and an estimated date the information will be available. Please note, however, that federal aid cannot be finalized until such time as all requested documentation is provided. Failure to satisfy all requests will result in the cancellation of all federal aid.

Documentation Required
  • A completed Verification form (available from the Financial Aid office).
  • Federal Transcript of Tax return (can be requested by filing out Form 4506-T and returning it to the Adult Education Office to have faxed, by visiting IRS.gov, or by calling 1-800-908-9946).

If corrections are required, the Financial Aid office will correct the data electronically with the Federal Student Aid Program and process your aid request.

Notification of Verification Results
Notification of your award eligibility and/or certification of federal loans will indicate your eligibility for federal funds. In cases where aid had been previously awarded, notification will only be sent when/if program eligibility changes.

Overpayment of Federal Funds
In cases where corrections result in an overpayment of federal funds, you will receive notice on steps required to repay the federal programs and the consequences of not making proper repayment.

Reporting Fraud

In cases where intent to commit fraud to obtain federal funds is suspected, the school will refer the case to the U.S. Department of Education's Office of Inspector General for review and resolution. No further processing of financial aid will occur until the case is resolved.

Effective January 2011

Copies of Federal Tax Form 1040, 1040A, or 1040EZ will no longer be accepted for verification purposes. Students selected for verification will have two options when verifying income:

1) Making a FAFSA correction and using the IRS Data Retrieval Tool
2) Submitting a Tax Transcript to the Financial Aid office

The quickest and easiest way to verify income is by using the IRS Data Retrieval Tool that is part of the FAFSA on the Web. If you have already completed your FAFSA, please return to FAFSA.gov and select “Make FAFSA Corrections”. Navigate to the Financial Information section and follow the instructions to determine if you are eligible to use the IRS Data Retrieval Tool to transfer your 2011 IRS income tax information into your FAFSA. Once your tax data is transferred please re-submit your FAFSA.

If you are not eligible to or wish not to use the IRS Data Retrieval Tool, then a Tax Transcript can be requested in one of three ways:

1) Completing Form 4506-T Request for Transcript of Tax Return
(It is recommended that “6a” is selected as it is the quickest option. It is also recommended that the document is returned toSanduskyCareerCenterto ensure it is faxed to the correct place. Please make sure to include all information required and sign and date the request form.)
2) Visiting IRS.gov to “Order a Transcript”
3) Calling 1-800-908-9946
