Transition services for children with disabilities are provided as part of the special education services written in the child's Individualized Education Program (IEP) beginning at age 14 or younger if appropriate, and continues until graduation from high school. A Transition Specialist communicates with the student's IEP Case Manager, to ensure that Transition Services and objectives are written in the IEP.
